Your right to a Good Faith Estimate

Understand expected costs before you receive health care.

You can request an estimate of your health care costs.

If you are uninsured or are not using certain types of health coverage to pay for care, federal law generally gives you the right to a written Good Faith Estimate of expected charges.

  • You may request an estimate before scheduling. It should include the expected cost of health care items and services, including related charges.
  • For care scheduled 3–9 business days ahead, the written estimate is due within 1 business day after scheduling.
  • For care scheduled at least 10 business days ahead, the written estimate is due within 3 business days after scheduling.
  • If you request an estimate before scheduling, it is due within 3 business days after your request.
  • If a provider’s or facility’s bill is at least $400 above its estimate, you may be able to dispute the bill. Keep a copy of your estimate.

Call (971) 227-6592 to request an estimate from Pacific Meridian Medicine. Listed visit prices are not a substitute for an individualized Good Faith Estimate.
